[Sticky] Help why are my fish dying? What we need to know to help you

 In order to help us answer your questions as quickly as possible please provide the following information (if you can) with your posts.
Aquarium size and shape.
Substrate type and depth.
Decorations/structures.
Type and size of filtration.
All of the fish and their sizes.
Live plant types and quantity.
How long the tank has been setup and is it cycled?(Cycling is covered in FAQs).
Lighting type and time on/off.
Feeding, food types, quantities, and frequency.
PH
Hardness
Ammonia
NitrIte
NitrAte
Salinity
Temperature
Water change schedule: how much and how often, when was your last water change?
Any symptoms, odd behaviors, or visual observations.
Any injuries the fish may have sustained and how.
Anything else you can think of that might help us help you. The more we know of your system the better we will be able to assist you so include as much information as possible in your post.

Also, might I add, that if you don't have the test kits, most fish stores will test your water for you for free.  If you can get test results there, you should try to do so.  Make sure you ask the store for the exact readings for each test they do.

Also, tell us if you have any new additions to your tank, such as new fish or new plants, new filters, etc - and when they were added.
